In partnership with Hampshire and Isle of Wight Healthcare NHS Foundation Trust our goal is to engage clients during their hospital stay, supporting their safe and efficient discharge from the wards. Our Hospital to Home Service is currently available in the following hospitals:
The Hospital to Home Service Aims...
- To improve the patients' experience of being in hospital and support their journey through rehabilitation and discharge.
- To provide activities to contribute to patients’ rehabilitation, health and wellbeing in preparation for discharge.
- To actively identify issues of patient concern regarding their discharge and work with the ward team to address issues to make for a 'good' discharge and to prevent re- admission.
- To provide information, signposting and referral to community support services as required.
- To make follow-up calls following patient discharge home, to ensure continued safety, health and wellbeing.

“Everything is doing well, I started my Neuro rehab, still waiting on the ENT appointment in October, had my check up in GUM clinic with the Doctor, but will go back to check after I finish the medicine. I hope soon I can go back to work after all the treatment, rehabilitation and check-ups and go back to driving as well.” Terry
